Abstract Background Although rare, spontaneous splenic rupture (SSR) is a potentially life threatening condition and most commonly associated with infection malignancy. Haematological malignancies are an important differential diagnosis among these chronic myeloid leukaemia (CML) has one of the highest associations SSR. Methods This case report follows previously fit healthy thirty-three year old gentleman who presented to accident emergency day history sudden onset, severe, left upper quadrant pain. CT abdomen pelvis including arterial phase, confirmed moderate volume haemo-peritoneum, but no active bleeding. White cell count (WCC) was 225.8 x10^9/l blood film suspicion haematological The patient haemodynamically stable therefore transferred surgical high dependency unit for observation conservative management. He commenced on hydroxycarbamide advice haematology. Results Genetic screening detected BCR-ABL1 Philadelphia chromosome; this along bone marrow aspirate CML. His WCC decreased therapy. made good clinical progress discharged home haematology follow up. Hydroxycarbamide since been stopped he initiated Imatinib returned normal remained well further complications.
